Clinical Diagnostic Assessment
Once you have completed the online application, please call our office and schedule an assessment. We will collaborate with other healthcare providers who may provide additional information in your CDA.
We will request the following documents with a signed Release of Information form found in the intake packet:
- Any assessments completed from other mental health providers or healthcare providers.
- Health and Physical-Requested your PCP
- Behavioral Health Assessments and Treatment Plans from schools if client is an adolescent.
What to expect during your CDA:
One of our therapists will meet with you to discuss your current goals of therapy. Our goal is to make your plan client-centered. We will go over current concerns and a history of what has been brought to our agency. Your therapist will also discuss our community-based programs that can work in conjunction with your therapy goals.
Assessing Services
Once you have completed the CDA, our therapist team will get to work on a therapy plan and the correct modality to serve your needs. You may be contacted by a members of our management manager if CBRS, CM, TCC, or Peer Support was recommended. We want this process to be easy on you and to start working towards your therapy goals together. We go at your pace and our thankful that you have chosen our agency.
All of our services have gone to a telemental/phonic platform with the stay-at-home order. To provide the safety of our team and members of the clinical services, we are using the following site doxy.me. The site is free to our members and most importantly HIPAA compliant.

Privacy Notice
All employees are required to maintain the confidentiality of your private health information. Your records and communications with employees will not be disclosed, even after you discontinue services, unless required by law. No third party is permitted to view your private records you agree in writing, or permitted by law. Your name and other basic identifying information are submitted via a computerized billing system for billing purposes. Certain persons authorized by the state government may view your private information for purposes of auditing, billing, and collection.
We must report any life endangering situation that comes to our attention. We are required by law to report any instances of child sexual abuse, physical abuse, or neglect. We must warn others about threats you make against them.
